Hypertension is an increasingly recognized health issue in the pediatric population. Unlike adults, in whom blood pressure staging relies on fixed absolute values (such as stage 1 beginning at 130/80 mmHg), pediatric blood pressure staging is highly dynamic. Normal values vary based on the child's sex, age, and height percentile, reflecting structural growth, vascular development, and hormonal changes.

In 2017, the American Academy of Pediatrics (AAP) published updated clinical practice guidelines for screening and managing high blood pressure in children and adolescents. These guidelines provide standardized staging to prevent cardiovascular organ damage and identify secondary causes early.

For children aged **1 to 13 years**, staging is determined by comparing systolic and diastolic blood pressure readings against reference tables based on age, sex, and height percentile:

Classification Systolic / Diastolic Thresholds (Ages 1 - 13 Years)
Normal Blood Pressure < 90th percentile
Elevated Blood Pressure ≥ 90th percentile to < 95th percentile,
OR 120/80 mmHg to < 95th percentile (whichever is lower).
Stage 1 Hypertension ≥ 95th percentile to < 95th percentile + 12 mmHg,
OR 130/80 to 139/89 mmHg (whichever is lower).
Stage 2 Hypertension ≥ 95th percentile + 12 mmHg,
OR ≥ 140/90 mmHg (whichever is lower).

3. The Clinical Importance of Height Percentiles

Height percentile acts as a proxy for physical maturity and vascular path length. Taller children have longer arterial trees, requiring higher pressure gradients to maintain systemic perfusion. Therefore, normal blood pressure is higher in taller children. Clinicians must first determine the child's height percentile (usually using CDC growth charts) before looking up blood pressure percentiles.

4. Investigating Secondary Hypertension

In pediatric medicine, primary (essential) hypertension is common in adolescents, especially when associated with obesity. However, in infants and young children, or children with Stage 2 hypertension, the probability of an underlying secondary cause is extremely high. Key etiologies include:

  1. Renal Parenchymal Disease: Glomerulonephritis, reflux nephropathy, polycystic kidney disease (the most common secondary cause overall).
  2. Renovascular Disease: Fibromuscular dysplasia, renal artery stenosis.
  3. Cardiovascular Disease: Coarctation of the aorta (manifests as a significant blood pressure difference between upper and lower extremities).
  4. Endocrine Etiologies: Pheochromocytoma, hyperaldosteronism, congenital adrenal hyperplasia (CAH).
